TESTING FOR URBAN MEMBERS
On the Saturdays of August 1, 2020, August 15, 2020, and August 29, 2020, the Gila River Indian Community (“Community”) will provide free Covid-19 Virus testing for urban Community members that reside off-Reservation in Maricopa County and Pinal County. If you have already signed-up through the portal system below to take a prior test, you do not need to sign up again to take the Covid-19 Virus tests during the month of August 2020. The Community encourages its Urban members to be take the test each time it is offered during the month of August 2020.
For those Urban members that have not previously signed up for testing, you will need to register for the test by clicking the portal button below and entering the requested information in the online form. Household family members who reside with an Urban member at the same address are eligible for the testing on the above dates. This includes household members that are not Community members. Information regarding household family members who will take part in the testing will need to be included in the online form provided in the portal below. If you have not previously signed up to take the Covid-19 Virus testing offered by the Community, you will need to register by 6:00 p.m. on the Tuesday prior to that week’s Saturday test date. Once you have signed up you do not need to re-enter your information for future testing dates unless you move to a new address or change your contact phone number.
Testing for Maricopa County residents on the dates of August 1, 2020, August 15, 2020, and August 29, 2020, will be held on the first floor of the Native American Community Services Center located at 4520 N Central Ave, Phoenix, AZ 85012. Testing begins at 8:00 a.m. and ends at noon, 12:00 p.m.
Testing for Pinal County residents on the dates of August 1, 2020, August 15, 2020, and August 29, 2020, will be held at the Revolution Cheer Company & Dance Studio located at 986 N. Mission Pkwy, Casa Grande, Arizona 85122. Testing begins at 8:00 a.m. and ends at noon, 12:00 p.m.
All persons being tested must bring their own masks and wear them at the test sites in order to protect their health and the health of others.
Each person will need to show a driver’s license, Community Member Identification Card, or otherofficial identification (unless they are minors under the age of 18) for the test. However, only children nine (9) years and older are allowed to be tested. Persons not being tested should not be brought to the testing sites.
Each person will need to sign a consent form for the test which will be provided at the test site. Information about receiving test results will be provided at both test sites.
Those persons that do not have access to a computer or smartphone to use the above portal may call the Community’s Consultant, the Center for Applied Research at 303-294-0678 to provide necessary information by phone. Information provided by phone must be received by 6:00 p.m.
on the Tuesday prior to that week’s Saturday test date.

Finally, please do not call Native American Connections or Revolution Cheer Company & Dance. While these two organizations have graciously allowed the Community to use their facilities for the testing sites, the Community hired a separate certified and licensed health care provider to administer the testing.
